COVID19 vaccine & City-id event in Northeast Philly with Chinese language support Date： Saturday 12/04/2021; 10 AM - 3 PM Location: Feast of Justice 3101 TYSON AVENUE, PHILADELPHIA, PA 19149 Provides Pfizer and Moderna 1st, 2nd, and Extra/Booster Dose of the COVID-19 Vaccine for people 12 year and older,   plus the special Pfizer COVID-19 vaccine for Children 5 - 11 years old.  Children 5 - 11 years old need to receive the 2nd dose  after 20 days and they can  come back   on 1/15/2022 at the same time to get it .  One  can make appointment using this URL:   https://bit.ly/3cowsb1         City ID service is also provided in the same event.  One  can make appointment   this URL:  bit.ly/3qegSXE Both activities required sign up by Google forms but we will not refuse walk-in.
